What makes a condom break? Is there a specific reason for it to break during sex or is it just bad luck?

During the past two weeks, the condom has broken twice already (two different times) and I want to know why did it break? And what can I do to avoid it breaking? This has never happened to me or my boyfriend before but it's already happened twice with us...And I don't know if this has anything to do with it, but it breaks after he comes, but before that, it's perfectly fine.

Please help me out!
